Data Recording Sheets
Below are a collection of sheets that may help you collect data for your HELPS and SPROUTS students. All forms are editable, so feel free to alter them to make them work for you. Please see Noah or Chris if you need help choosing a sheet for your particular case.

Good weekly tally sheet with AM and PM sections for all days.
Daily/hourly behavior sheet with rooms to write notes.
A sheet to monitor times a student may have to leave the room for behavior issues. Good documentation for students that have to leave often with Mrs. Davis.
A good baseline sheet for behavior data. Useful when not sure what behavior to target first.
A more precise tool for measuring duration of behavior.
A more compact version of the above tool.
Although written for checking off "following directions," This form can be modified to be used for tracking a wide variety of behaviors. This is one of my "go-to" sheets for easy data collection.
An all-in-one, day-by-day, monthly behavior data collection and graph. Many teachers really like this one!
A good peer comparison data collection tool.
Another good tool for keeping track of the time your student is in and out of your room.
A good tool for keeping track of more than one student at a time to track data. Can be modified to address a wide variety of behaviors.
